These third parties will advise to us of your eligibility to rebates on eye care services and eyewear products.

In providing you with our services we need to collect personal information from you, which is necessary for us to perform a thorough service and provide you with the most suitable eye care. Your Lowes & Partners optometrist has access to your records for professional purposes only.

• advise you on your eye care needs including when you need another eye exam.
• seek your input on your service from Lowes & Partners.
• keep you updated with new eyewear products and services and special promotions and offers.

Our optometrists, with your consent, may refer you to an eye specialist and forward relevant information to that medical practitioner about your particular condition.
